I need an interpreter: Spanish ASL other: _____________________
I, Plaintiff, claim that on or about (date) 12/30/2024 , the above-named defendants owed me the sum
of (or property valued at) $_____________
$10000.00 because:
The defendant, Bricks & Minifigs Keizer, received LEGO sets and minifigures under a consignment
agreement. The store agreed to sell the items and pay me the agreed-upon proceeds. Instead, the defendant
kept my property for months, refused to return it when requested, and did not pay me for the items they sold.
The defendant repeatedly changed explanations about where my property was, claimed items were “lost,”
and then stopped responding to all communication. Because of this, I was unable to recover my property or
receive payment. I calculated my damages based on the fair market value of the LEGO items I consigned,
using average sale values from BrickLink, eBay sold listings, and Bricks & Minifigs’ own pricing.
The defendant’s actions resulted in financial loss equal to the value of the missing/withheld items. I am
requesting the amount shown in my claim as compensation for the property the defendant kept and/or the
money they failed to pay me.
,
and this amount is still due.
I have paid (or will pay): Claim $______________
$10000.00

DECLARATION OF GOOD FAITH EFFORT
I, Plaintiff, have made a good faith effort to collect this claim from the defendants before filing this claim
with the court clerk. (Describe your efforts):
I have repeatedly tried to resolve this matter directly with Bricks & Minifigs. I called the store many times,
spoke with staff and management, and even went in person to request the return of my LEGO sets. Each
time, they promised to return them or arrange payment but kept postponing it. It has now been over a
year, and they still have not followed through. They have not resolved this issue.
See the Oregon Judicial Department website for information about debt collection cases:
www.courts.oregon.gov/debtcollection
I have complied with ORS 646A.670(1) and UTCR 5.180. A completed Collection by a Debt Buyer
Disclosure Statement is attached and incorporated into this pleading as required by UTCR 5.180(2)(b)
(iii).
I hereby declare that the above statements are true to the best of my knowledge
and belief. I understand they are made for use in court and I am subject to
penalty for perjury.
